Our Intake Packet: Failure to complete your intake packet 24 hours before your appointment will result in your appointment being cancelled. All clients must have a valid credit card on file in order to schedule an appointment.

Welcome Message/ Insurance Verification: You will receive an email within 24-48 hours of requesting an appointment with insurance details, fees, and out of pocket costs
